In our podiatrist opinion the best metatarsalgia shoe insoles are a full length well fitted insole.
A custom orthotic for metatarsalgia is the best option if possible with your podiatrist. This type of custom insole can have a molded metatarsal pad built into it.

Best Running Shoes for Metatarsalgia Pain:
What makes the best running shoes for metatarsalgia?
There are 3 main types of foot classication associated with metatarsalgia. They can range from over supination (high arched foot) to overpronation (flat foot).
The APMA states that these different foot types would benefit from different shoes.
This gets more confusion for metatarsalgia because both flat feet and high arch feet can have ball of the foot pain.
Good running shoes for metatarsalgia:
The APMA states that a good running shoe for metatarsalgia may include 3 components.
First find out whether you have a high arch, a neutral arch or a normal arch.
A high arch shoe should offer more flexibility and a softer sole. This softer sole that is more flexible will compensate for the poor flexibility and shock absorption of the high arched foot. This helps correct for supination
The foot with a low arch or that is flat footed should focus more on stability. Stability and motion control are important for people with flat feet because this helps correct for overpronation.
People with more of a normal arch: If you have a normal arch then you can stick with a more neutral high quality shoe. This is a balance of stability and cushioning to help absorb shock for your foot.
