The most common causes of top of the foot pain:
- Top of the foot nerve compression.

Pain On Top Of Foot Overview:
- Pain on the top of the foot is widespread. We see this almost every single day in our clinic.
- This is commonly combined with bottom of the foot and heel pain called plantar fasciitis or ligament damage.
- The mechanism of action is that the bone on top of the foot compresses, while the ligaments and plantar fasciitis at the bottom of the foot elongate and tear.
- This causes compression, cartilage, and bone damage on top of the foot. While on the bottom of the foot, the ligaments and plantar fascia container and get microscopic damage throughout the ligaments.
- This leads to chronic and crippling foot pain that some people stop walking to.
- Even before surgery, there are many treatments, although surgery can be very effective in relieving the top of the foot bone spur.
- If a bone spur is left to continue growing, it can cause the tingling top of the foot pain by irritating the top of the foot nerves.
Symptoms:
The most common symptoms of top of the foot pain are:
- Sharp, shooting, and tingling pain.
- This pain usually gets worse while you’re standing and walking during the day.
- The less supportive the shoes, the more your foot pain will hurt.
- The pain is usually where there is a bony prominence on top of your foot.
Most Common Causes:
1) Dorsal Compression Syndrome:
- Dorsal compression syndrome occurs when there is too much stress on the ball of your foot.
- This compresses the top of the foot.
- It stretches out the bottom of the foot.
- So if the top of your foot is numb or irritated & the bottom is in pain, this is it!
- This can also lead to a lack of shock absorption in walking or running.
- It will eventually lead to mid-foot arthritis.
2) Top Of Foot Numbness Or Tingling:
- The top of the foot tingling is usually due to nerve irritation.
- The medial dorsal cutaneous nerve is most frequently affected.
- This nerve covers the bump on top of your foot and runs along the top of your big toe.
- If a nerve is irritated and becomes numb, this is known as neuropraxia.
- Neuropraxia can take up to a month to get better, so treat it properly!
3)Top Of The Foot Bone Spur:
A bone spur on top of the foot forms due to abnormal friction and stress on the bone.
- As the bones of the midfoot rub against each other, the bone re-enforces itself.
- This is similar to a callus forming in a high-pressure area.

5)Sprained Top Of The Foot:
- A sprain in the top of the foot or the arch is called a Lisfranc sprain.
- In most cases, it is usually only a sprain that lasts a couple of days to a couple of weeks.
- But in some cases, it may be essential to seek imaging & further treatment.
6)Fluid Cyst:
- Ganglionic cysts are soft masses across the top of the foot which are not immediately dangerous!
- These are simply pockets of fluid that pouch out from the joints of your foot bones like a balloon.
- Treatment consists of draining and removing the pocket of fluid.
- But they should still check out by a podiatrist because there is a chance that they may be something a little more dangerous.
7) Extensor Tendonitis:
- Just like a nerve can get irritated on top of the foot, so can tendons.
- Extensor tendons are the ones responsible for lifting your foot toward your shin.
- They can become irritated from being overworked.
- This is very common in long-distance runners and running hills.
- The front of your leg and the top of your foot will be sore.
8)Plantar Fasciitis – Entire Arch Pain.
- This is by far the most common pain in the arch of your foot.

Most Important Tips For Orthotics:
- Make sure you have a roomy enough shoe.
- I’m warning you right now, don’t try to stuff a full-length orthotic into a tiny tight shoe. It won’t work.
- If you have tighter or dress shoes: try the dress shoe or 3/4″ orthotics.

- If you are tight in the front of your shoes, or the shoes are tighter, these 3/4″ might be the best choice for you.
- But if you have a choice, the full-length orthotics are much, much more supportive!

Full-length orthotics give you the most correction and improvement:
- If you have roomy enough shoes like running shoes or work boots. Get a full-length orthotic.
- The cushion under the front of your foot prevents it from twisting out against the outside of your shoe as much as possible.
- These are one of the best possible options for the medium and heavy-duty correction!
Dress Shoe orthotics:
- These are premium leather orthotic recommendations for dress shoes.
- If you have a tight dress shoe get the 3/4″ orthotic.
Surgery for the top of the foot pain:
It is possible to use surgery to excise the bone spur that develops on top of the foot:
- This can be a longer recovery time, taking roughly 2 to 3 months to get back to even 75 to 80%.
- This does work very well, but the key is to keep wearing good inserts and good shoes.
- You have to consider that as you’ve gotten older, the muscle mass and the flexibility in your body have decreased.
- If you’re developing arthritis in an area, this likely means that your body is compensating through this area.
- Unfortunately, even after surgery and cleaning of the spur, you are not a teenager again.
- This means the same underlying causes that created this pain are likely developing still, and it is only a matter of time before new spurs start to form.
- The best way to guarantee that this will not happen again is to get in as good shape as possible, lose weight, get great shoes, get great orthotics, and this is where we see the best possible results.
Top of the Foot Home Treatment:
- There are usually two phases to treating the top of the foot pain treatment.
- This doesn’t matter whether it’s an intermittent sharp pain on top of the foot, pain on top of the foot near toes, tendonitis on top of the foot, or a top of the foot bone spur. Treatment is all roughly very similar.

Top of the Foot Pain Trauma:
- If you have a sprain Of the foot, Lisfranc fracture, or a fracture to the top of the foot, you need to protect it.
- The recommended way of doing this is to offload with a good supportive boot or cast.
- The best way to do this is, of course, to see your podiatrist and get evaluated with an x-ray, ultrasound, and potentially even an MRI or CT scan.
- If you cannot do so, it may benefit you to be in a cast, fractured boot, or even keep the weight off of it with a rolling knee scooter or other protective devices.
- We as podiatrists frequently take patients off work for a very long period of time when they suffer a traumatic injury. Unfortunately, there is no other way around us in labor jobs.
- If you have a sit-down job, there are ways to get people back to work quicker, but this can be impossible otherwise.
Top of the Foot Fracture Boot Treatment:
- There are pros and cons to using a boot to treat your foot pain. The pros are that your injured heel will hopefully have a chance to heal gradually! If you are immobilized too long, the cons are that you will gradually become stiff and overworked to your other leg. r

- If your Achilles tendonitis pain is severe, offloading can be very effective until the pain calms down.
